Admission Requirements for Medicare Patients
A PASRR from the discharging hospital or facility must be received prior to admission.

Steps to Admission at Vibra Hospital (LTAC) and Vista View Care Center (SNF)
Thank you for your interest in Vibra Hospital and/or Vista View Care Center. We value that you have entrusted the care of you or your loved one to us. Our goal is to provide a comprehensive individualized plan of care for each patient to reach their maximum recovery potential. Our staff works closely with patients, their families, physicians, case managers, insurers and other healthcare providers to ensure a smooth and timely coordination of care.
Please call 303-286-5000 to refer patients to Vibra Hospital (LTAC) or Vista View Care Center (SNF). Referrals are accepted 24 hours a day, seven days per week.

Upon referral to Vibra, a clinical liaison will complete a pre-admission screening for each patient and an admission coordinator will contact your insurance company or Medicare about your eligibility for benefits. Vibra is a contracted participating provider with Medicare, Medicaid, managed care organizations and insurance companies. The Vibra team will then review this information based on our admission criteria and determine the appropriate level of care to meet you or your family member's treatment needs. Vibra's commitment is to advise the referring facility/case manager of the patient's acceptance or denial within a two hour period.

Families of patients are asked to call our admissions department at 303-286-5000 to schedule a time to come in to sign consent to treat and other necessary admission paperwork prior to or at time of admission.

At Time of Admission to the Hospital, Please Bring the Following Items:
  • Insurance Cards (Including Medicare and/or Medicaid)
  • Living Will or Advance Directive (if Applicable)
  • Medical Durable Power of Attorney or Guardianship Papers (if Applicable)
